Introduction: Acute leukemia is a common malignancy in childhood that is

now curable with chemotherapy.Chemotherapeutic agents have numerous complications. We studied the incidence of GI complications and its relations

with specific phases of treatment.

Material & methods: In a cross-sectional study, we examined 75 patients

with acute leukemias who were under chemotherapy (oct 2003-aug 2004) in

Sheikh Hospital ,Mashad University of Medical Sciences,IR Iran. These patients

had at least one GI complication caused by chemotherapy. The data was

collected using a questionaire and the statistical analysis was performed.

Results: In 75 patients the mean age was 7.3 years with the most age distribution between 5-7 years. Of them 23 were female and 52 male. The types of leukemia in these patients were: 62 patients acute lymphoblastic leukemia(ALL)

The B-cell variety, 3 cases T-cell ALL and 10 Acute myeloblastic leukemia

(AML). The most common complication was oral ulcers in 42 cases. Other complications were : nausea & vomiting 35, diarrhea 20 who had negative cultures, odinophagia 19 , candidiasis 16. Abdominal pain 12 , abdominal distension 10, perianal fistula & ulcers 8 , constipation 5,hematochezia 3, melena 2 and dysphagia 1 case. Perioral ulcers,diarrhea, nausea and vomiting and oral aphthous ulcers were notably more common during the first phase of acute lymphoblastic leukemia treatment. Typhlitis was not seen in our patients.

Conclusion: The most common GI complications of chemotherapy in childhood leukemia were : oral ulcers ,nausea & vomiting ,diarrhea ,odinophagia,candidiasis, abdominal pain,abdominal distension ,perianal fistula & ulcers,constipation hematochezia melena and dysphagia.

Chronic Liver Diseases in Children and Oral Manifestations

INTRODUCTION:Cirrhosis and other chronic liver diseases are detected sometimes with oral manifestations. Some of these complications are life threatening.

MATERIALS &METHODS:In this prospectivecase-control study 31 patients (8 females ,23 males) with chronic liver diseases were studied during one year.

RESULTS :The mean age was 9.5 years. Four cases were younger than five years old, 12 cases between 5-10 years and 15 cases older than 10 years.

The causes of chronic liver diseases were: Idiopathic cirrhosis (15 cases), Wilson’s Disease (4), chronic active hepatitis (3), chronic persistant hepatitis (2), congenital fibrosis (2), congenital lipodystrophy (1), cirrhosis following neonatal hepatitis (1), polycystic disease (10), hepatitis C cirrhosis (1), portal hypertension following thrombosis (1).Oral manifestations of chronic liver diseases were studied.

CONCLUSION :The most common oral manifestationswere: gingival hemorrhage (87%),glossitis (48.3%),cheilitis (32.2%), jaundice (32.2%), lip pigmentation (16.1%) and dry mouth (3.2%).

مقایسه تاثیر آهن درمانی خوراکی و وریدی در بیماران همودیالیزی تحت درمان با r-HUEPO

مقدمه و هدف: آنمی یکی از عوارض بیماری کلیوی و همودیالیزی در بیماران همودیالیزی مبتلا به CRF و ESRD است .درمان جایگزین با r-HUEPO با اصلاح نمودن این عارضه کیفیت زندگی بیمار را بهبود بخشیده و نیاز به ترانسفورزیونهای مکرر را کاهش می دهد . اما کمبود آهن ناشی از همودیالیزی پاسخ درمانی به r-HUEPO را محدود کرده و بیمار را نیازمند دوزه های بالاتر و تزریق مکرر این دارو برای حفظ و نگهداری سطح مطلوب هماتوکریت و هموگلوبین خون می نماید. هدف از این مطالعه مقایسه تاثیر آهن وریدی در برابر مکملهای آهن خوراکی در بیماران همودیالیزی مبتلا به CRF و ESRD تحت درمان با r-HUEPO می باشد .

مواد و روشها: در این مطالعه تجربی که بر روی 40 بیمار در محدوده سنی 21-5 سال با مدت همودیالیزی بیش از 2ماه (با میانگین22 ماه) انجام گرفت بیماران به دو گروه 20 نفره تقسیم شدند. دو گروه از نظر متغییرهای تاثیر گذار همچون سن، جنس، ... همسان سازی شدند و تفاوت معنی داری میان آنها مشاهده نشده است . به گروه اول آهن وریدی dextran و به گروه دوم آهن مکمل خوراکی Ferrous Fumarant به مدت 20 هفته دریافت نمودند . اطلاعات به دست آمده با استفاده از آزمون آماری t-test مورد سنجش قرار گرفتند .

نتایج: یافته ها نشان داد که در گروه دریافت کننده آهن وریدی افزایش قابل ملاحظه فریتین سرم و (reticulocyte Hb Contetnt) CHR و نیز کاهش دوزr-HUEPO در مقایسه با گروه دوم مشاهده گردید . (02/0 P-V<) . در هر دو گروه هماتوکریت ثابت باقی مانده بود و نیاز به ترانسفورزیون خون در هیچ یک از گروه ها مشاهده نگردید .

بحث و نتیجه گیری : ناتوانی در جذب و ذخیره سازی مقادیر کافی آهن از مکملهای خوراکی و همچنین عوارض گوارشی آن مانند تهوع و استفراغ ، استفاده طولانی از آنها را محدود مینماید .برخلاف آهن خوراکی، آهن وریدی افزایش قابل توجهی در میزان فریتین سرم ایجاد کرد . لذا تجویز آهن وریدی می تواند یک روش جایگزین جهت ذخیره سازی ، حفظ آهن سرم و کاهش دوز r-HUEPO مورد نیاز باشد . مطالعات بیشتر مورد نیاز است تا دوز مطلوب و فواصل درمانی را ارزیابی نموده و اثرات طولانی مدت آهن وریدی را با توجه به فرهنگها ، عادات غذایی و سیستمهای مراقبت از سلامت متفاوت بررسی نماید.

نتایج حاصل از عمل جراحی طحال برداری در بیماران بتاتالاسمی ماژور

خلاصه: بیماران مبتلا به تالاسمی بعلت نقص ارثی در زنجیره گلوبین با ناپایداری گلبولهای قرمز نیازمند تزریق خون مکرر و بدنبال آن طحال برداری پیدا می کنند . هدف از این مطالعه بررسی نتایج عمل جراحی طحال برداری در این بیماران می باشد .

این بررسی بر روی 45 بیمار مبتلا به تالاسمی که در طی ده سال گذشته (1379-1369) عمل جراحی طحال برداری شده بودند و جهت پیگیری به مرکز تالاسمی بیمارستان شفا مراجعه می کردند به صورت توصیفی و گذشته نگر انجام گرفت . از بین 45 بیمار 28 نفر (2/62%) تالاسمی ماژور و 17 نفر (8/37%) تالاسمی اینترمدیت داشتندو اکثریت آنها بالای 15 سال بودند .در 60% موارد طحال تا حد ناف یا بزرگتر از حد ناف، در 7 بیمار (5/15%) سنگ کیسه صفراوی همزمان و در 11 بیمار 5/24% درد فضای ربع فوقانی چپ شکم و سیری زودرس داشتند . 43 بیمار (5/95%) ماهیانه بیش از 20 سی سی به ازا هر بیمار 5 کیلوگرم وزن بدن خون می گرفتند. بعد از عمل جراحی طحال برداری23 بیمار (5/57%) ماهیانه بین یک ماه تا یک سال بعد از عمل و 6 بیمار (15%) بیش از یک سال بعد از عمل نیاز به اولین خونگیری داشته و میزان و دفعات خونگیری نیز کاهش یافت و تمام بیماران احساس راحتی و تحرک بهتری پیدا کردند .3 بیمار (66/6%) دچار عارضه جراحی شده و علی رغم دریافت واکسم پنوموواکس، قبل از عمل 9 بیمار (20%) ، دچار عوارض عفونی در طولانی مدت شدند .

عمل جراحی طحال برداری با کاهش نیاز به خونگیری و پیشگیری از عوارض حاصل از آن رفع علائم بزرگی طحال در این بیماران روش درمانی مناسبی است معهذا با توجه به عفونی حاصل از عمل جراحی طحال برداری (20%) در این بیماران عمل جراحی طحال برداری فقط در مواردی که خونگیری مکرر و زیاد بیمار باعث افزایش بروز عوارض خونگیری (هموکروماتوز) شده و یا بزرگی وسیع و علامتدار بیمار پیدا می کند توصیه می شود.

Indication of partial versus complete splenectomy for decreasing need to transfusion

Introduction : splenomeglay is a significant clinical finding before starting blood transfusion in thalassemic patients, also is usual in spherocytosis, ovalocytosis , storage diseases like Gaucher s and Neimann Pick . But after second to third transfusion spleen is not palpable in thalassemia major. In some other hemolytic anemias like Pyrovate kinase deficiency splenomegaly may be significant . Because of important role of spleen in immunity total splenectomy may be associated with sever post splenectomy sepsis, so partial splenectomy is a good alternative for it.

Case histories: in last 10 years(1996-2006) partial splenectomy was requested for 84 thalassemia major, 16 thalassemia intermadia, 9 Pyrovate Kinase deficiency, 3 Gaucher s disease. Five percent developed splenomegaly 5-7 years after partial splenecyomy , but no one needed total splenectomy (i.e. second operation). In the same period total splenectomy was requested for 140 cases of thalassemic patient, 3 patients developed increased need to transfusion due to prophylactic penicillin therapy (with Hapten mechanism), 3 cases developed sever Eosinophilia, one case developed increased need to transfusion due to enlargement of accessory spleen.

Discussion: In the cases of hypersplenism after 5 years of age, whose Coomb s test is negative and B.M is not aplastic then there is question about versus partial splenectomy.

B.M is not aplastic then there is a question about total versus parial splenectomy.

Red pulps of the Spleen are site of storage of iron, and white puples are site of production of Opsonins, Properdin factor and antibodies, so total splenectomy may be complic ated by sever life threatening infections. Prophylactic P.O Penicillin therapy is indicated after total splenectomy and may not substituted with monthly Penador injections (because it is sufficient for Streptococcal infection but not for Pneumococal infections).

Sever Eosinophilia, Leufler syndrome and hemolytic anemia (by Hapten mechanism) are some of common complications of prophylactitic penicillin therapy.

There is another question about need for reoperation after partial splenectomy , in my experience if partial splenectomy is done by expert surgeon less than 5% of the patients may have significant splenomegaly, and most of them have no sign of hypersplenism.

THROMBOCYTOSIS which is a known complication of total splenectomy is rare (and always transient) in partial splenectomy.

So if there is not any absolute indication for total splenectomy (like Coomb s positive hemolytic anemia), partial splenectomy is advisable.

In the case of Isoimmunisation who response to medical therapy (steroid, I.V.IGg , …) only total splenectomy is indication for accessory polyvalent vaccine and Sonography before surgery and surgical exploration for accessory spleen is indicated In the cases of hypersplenism before 5 years of age(thalassemia intermedia, congenital spherocytosis, Pyrovate kinase deficiency, some types of Gaucher s and Neiman Pick disease ) partial splenectomy is indicated.

IN OSTEOPETROSIS (Marble bone disease) splenectomy is contraindicated because they have no bone marrow cavity, so spleen is the major organ for hematopoiesis
